SCHEDULING AND REGISTRATION INFORMATION
You must pre-register for all classes. To register for a class, return the enclosed registration form along with your company check (All personnel check must have Driver License number, address, home and work phone number), major credit card number and expiration date or money order to: Chapman Health Kare Corporation, P.O. Box 44530, Ft. Washington, MD 20749

REFUNDS AND CANCELLATION
* NO REFUNDS. You are entitled to a full refund ONLY if CHKC cancels a class.
* Once registered for a class, if you are unable to attend, (after proper notification), you are allowed to reschedule once within two months. If you need to take the class over you can take the class once within 60 days...NO REFUNDS.
